Telehealth billing for therapy and psychiatry practices
Billing for virtual-only and hybrid mental health practices.

How telehealth mental health billing works
Telehealth claims usually use the same CPT codes as in-person visits. What changes is the place of service and the modifier. POS 10 is used when the patient is at home and POS 02 when the patient is somewhere other than home. Many plans require modifier 95 for audio-video visits and modifier 93 for audio-only visits.

Medicare’s telehealth rules have changed several times in recent years, including its in-person visit requirements for mental health telehealth, so we check the rule in effect on each date of service.
Codes we bill every day
| Code | What it covers |
|---|---|
| POS 10 | Telehealth with the patient in their home |
| POS 02 | Telehealth with the patient somewhere other than home |
| Modifier 95 | Synchronous audio-video telehealth (plan-specific) |
| Modifier 93 | Audio-only telehealth (plan-specific) |
| 90791 / 90837 | Intake and therapy, same codes as in person |
| 99212–99215 | Psychiatric medication management by video |
Code descriptions are summarized. Payer coverage and documentation rules vary by plan and state.
Denials we prevent
- Wrong POS for where the patient was
- Missing or wrong telehealth modifier for that payer
- Provider not licensed in the state where the patient was located
- Audio-only visits billed to a plan that does not cover them
- Plans that require a specific telehealth platform or consent
Payer rules to watch
The provider must be licensed in the state where the patient is located at the time of the visit, not where the provider sits. For multi-state practices, we track licenses and payer enrollment by state.
Questions about telehealth mental health billing
Can we see patients in other states?
Only if the provider is licensed in the patient’s state, or the state allows it under a compact or exception. We check before you schedule.
Do you bill audio-only visits?
Yes, for plans that cover them, with the modifier and codes each plan requires.
