How we handle it
Review
Read the note, orders and results.
Code
Assign CPT, ICD-10-CM, HCPCS and modifiers.
Check
Run NCCI, MUE and payer policy edits.
Feedback
Send documentation tips to providers when needed.
Details that matter
Code sets change on a fixed schedule: ICD-10-CM updates every October 1 and CPT updates every January 1, with quarterly HCPCS and NCCI updates in between. Office and outpatient E/M visits (99202–99215) are leveled by medical decision making or total time on the date of service.
We code to what is documented. When a note does not support the level or service, we tell the provider what is missing instead of guessing.
Questions about medical coding
Do you code from the note or from a superbill?
Either. We can code fully from documentation or validate the codes your providers select.
Will you change our providers’ codes?
We flag mismatches and explain why. Final coding follows documentation and your practice policy.
