Orthotics and prosthetics billing with L-codes done right
Billing for O&P practices: custom and off-the-shelf L-codes, functional levels for prosthetics, right and left modifiers and prior authorization for selected items.

How orthotics and prosthetics billing works
O&P items are billed with HCPCS L-codes. For lower limb prostheses, Medicare coverage depends on the patient’s documented functional level (K0–K4), which must be supported in the prosthetist’s and physician’s records.

Off-the-shelf orthoses, custom-fitted orthoses and custom-fabricated devices are coded differently and require different documentation.
Codes we bill every day
| Code | What it covers |
|---|---|
| L-codes | Orthotic and prosthetic devices and components |
| K0–K4 | Functional level modifiers for lower limb prostheses |
| RT / LT | Right and left side |
| KX | LCD documentation requirements met |
Code descriptions are summarized. Payer coverage and documentation rules vary by plan and state.
Denials we prevent
- Functional level not supported by physician records
- Custom-fabricated codes without documentation of fabrication
- Missing prior authorization for selected orthoses
- Wrong side modifier
- Replacement within useful life without justification
